1.  Call our office to make your appointment.

2.  Print the appropriate forms found to the left of these

    instructions.

​

3.  Fill out all of the forms PRIOR to your visit.  If you do not      complete them ahead of time, it will take away from your        time with the doctor.  

​

4.  Bring the completed forms and any medications you take         with you to the office.  Don't forget to bring your                       insurance card.  Some insurances require you to select

     Dr. Rosen as your PCP before the visit.  If you neglect

     to do this, your insurance may not pay for the visit.

5.  If you have a co-pay, we accept Visa and MasterCard, but

    prefer cash or a check.  If you have a deductible, payment is

    due at the time of your visit.

​

6.  If you are sick, please wear a mask.

​

7.  If you cannot keep your appointment, please notify us as

    soon as possible so we can make the time available to

    another patient.  Remember, we run ON TIME and do not

    double book ! !
